As you age, sometimes you may need additional recovery time when you are ready to leave hospital, and additional time to make a decision regarding the best place to live in the long term. This type of care given to you when you are recovering is called transition care.
Transition care is a short term care programme that may involve low-intensity therapy such as Physiotherapy, nursing care (e.g. wound care) or personal care.
You may be cared for in your home or in some cases you may need extra support in an aged care home or a health facility that is separated from the hospital. Transition care is not intended to be delivered in a hospital type of environment.
Transition care can be provided for a period of up to 12 weeks. The programme might be extended to 18 weeks if you have been assessed as requiring more time for care.

The Australian Government might subsidise transition care however you will be expected to contribute towards your care financially if you are able to do so.
