Search for a Service Provider on the My Aged Care website at www.myagedcare.gov.au or by calling My Aged Care on 1800 200 422.

Speak with a Home Care Package Manager

  • When you contact a provider, you will be directed to a Home Care Package Manager or Coordinator.
  • You can arrange a meeting with the package manager to discuss a care plan and take the opportunity to explore what their fees, terms and conditions are, and what services you are able to receive.

Shop around for a service provider

  • You might like to meet with more than one service provider to discuss your options.
  • Different providers may offer different services that better suit your needs, and the fees may be different too.

Make a list of your care needs and services that you require

  • Before meeting the Home Care Package Manager, you might like to make a list of your care needs and services that you require to allow you to stay in your own home. That way you can discuss this at your initial meeting and find out if you can include these services into your package of care.

Construct a care plan with the Home Care Package Manager

  • Once you have decided on a service provider, you will be asked to construct a care plan with the Home Care Package Manager. This will specify what services/ equipment you are going to access and on which days.
  • The care plan can be very specific and it is intended to be directed by you, the consumer to meet your individual care needs. It is called a “Consumer Directed Care Plan”.
  • Once you have a care plan your provider needs to provide you with an individualised budget which sets out the total amount of funds and how those funds will be allocated.

Sign a “Home care agreement”

  • The service provider will also ask you to sign a “Home care agreement”. This is the legally binding document that you must sign in order to enter into a home care package with a service provider. It will contain all necessary information including fees such as exit fees.
  • It is highly recommended that you read the home care agreement prior to signing it, and that you seek legal advice as well as discuss the agreement with your friends and family.
  • Your service provider also needs to give you a copy of the Charter of Care Recipients’ Rights and Responsibilities – home care.
